Medal is 4 inches long, 1.5 inches wide. Engraved on reverse to Louis Stern. In the original box by the maker of the medal, Dieges & Clust. Doing an internet search, two matches for Louis Stern appear regarding the Civil War. The first, shows a Louis Stern was a member of a New York regiment. The second, is a New York Times article from May 8, 1905, where a Louis Stern spoke at the ceremony dedicating the war memorial that is depicted on the medal. In incredible condition, having never been cleaned, so the front has this great matte finish and the reverse is dark except for where the engraving of his name was done. I was given the following information by an "expert" regarding this medal.

He stated that I have an extremely rare, likely unique example that is "important", for these reasons: 1Of the few known examples of this very rare medal (there are no records of this medal ever appearing for sale anywhere, auctions included), the top bar reads "COMRADE". On my example, it reads "HONORARY MEMBER". There are no examples of this medal known where it has these words, and not "comrade".

2The other examples are not engraved with the persons name to who it was presented to, so unless there are iron-clad documents that can prove who it was awarded to, there is no way to know who it was given to. This medal appears to be given to the Louis Stern who is mentioned in the 1905 New York Times article.
