In doing research I have found 2 other very similar trepanning kits by Tiemann that were made for the USA Hospital Department, and are marked as such. This kit could have been a very early manufactured kit slated for government use, or as a private kit for surgeons. The odd thing is that the brass cartouche on this kit is blank, and we don't know if there is something engraved on the other side or not. Some of the USA Hospital kits G Tiemann manufactured did NOT all have sliding latches, and wrapped brass corners, etc.
It seemed like the earlier ones were the odd ones from what I have researched. One even had USA Hosp't Dept hand written on the cartouche but was deemed to be authentic by experts. We are not implying, nor could we imply that this kit was a government issued kit, unless something is found on the flip side of the cartouche.These sets are ultra rare to find complete, and in this kind of condition.
